OUR SERVICES
We are, and have been since 1973, the community’s only locally owned and operated comprehensive provider of Home Health, HOSPICE, and Private Duty services. This gives you unique access to a broad spectrum of home health and wellness services that allow seamless transitions between these services in your own home, assisted living facility, nursing home, etc.
Home Health
Skilled Home Healthcare is a range of services, ordered by your doctor and provided in your home. It’s seamless care: a transition from a physician's office or hospital to home, from illness to health, from troubled times to peace.
Hospice
Hospice is a Team of Healthcare Professionals and dedicated/trained Volunteers who can provide medical, emotional and spiritual care to individuals with life-limiting diagnoses, and to their family, friends, and caregivers.
Private Duty
If you’re just home from the hospital or seeking an alternative to nursing home or assisted living placement, U.P. Private Duty Services can assist in maintaining your level of independence.
