Hospice Philosophical Care

Hospice is a Team of Healthcare Professionals and dedicated/trained Volunteers who can provide medical, emotional and spiritual care to individuals with life-limiting diagnoses, and to their family, friends, and caregivers. The patient and family control “how much” or “how little” care is provided. Click here to learn more.

Service can be provided in most any setting or in a combination of settings – depending on the level of care wanted/needed:

  • In your “home” wherever that may be
  • Assisted Living Facility
  • Skilled Nursing Facility
  • Hospital

Anybody can make a referral.  Call (906) 225-4545 to speak with a qualified member of our staff or to request a FREE informational visit to find out how hospice can help you or your loved one. You are not required to enroll in hospice at that time.

Hospice is covered under the Medicare Hospice Benefit. This coverage includes visits by a nurse, physician, social worker, bereavement support, and other members of the hospice team. It also includes therapy, medication, and supplies.

While Medicare covers approximately 90% of all patients in hospice, most private insurances have a hospice benefit, as well. For uncovered expenses or comfort services, our patients and families may also request support from our nonprofit U.P. Hospice Foundation, where 100% of all donations go directly to patients and families and educating the community. NO eligible patient has ever been turned away regardless of their ability to pay.

U.P. Home Health & Hospice believes in Hospice Philosophical Care, which focuses on LIVING with a life limiting diagnosis. We encourage anybody diagnosed with a life-limiting diagnosis to schedule a FREE informational visit to discuss options EARLY so that we can discuss how hospice focuses on palliation (symptom management), education (on your disease path), empowerment (to make the choices that are right for you), priorities (for you to set during your diagnosis).  There is no requirement to sign up for hospice at that time, but patients find that the EARLIER they find out about their options, the better hospice can benefit them and their families.

It is important to know that you will still work with YOUR doctor, but we have a hospice team of professionals there to support you, you well.  The team consists of:

  • Hospice Medical Director
  • Clinical Manager
  • Nurses
  • Care Aides
  • Social Workers
  • Bereavement Counselor
  • Faith Leaders (if requested)

Our team addresses every need of the patient, family, and caregivers at regularly scheduled Interdisciplinary Team meeting (IDT). The members review and initiate any necessary interventions regarding medical, palliative, psychosocial, emotional, and/or spiritual needs.  The IDT meets at least every two weeks to discuss patient and family goals, requests, and suggestions. The team will review every facet of the patient’s and family’s comfort and care.  Other IDT supports include dietitians, pharmacists, counselors, and volunteers.

Bereavement and grief support is an important part of the Hospice benefit. It includes a number of options including individual and group bereavement support, twice annual memorials, spiritual care (if desired), and a number of different services for 13 months after the passing of your loved one.
