Serving Veterans Through Hospice Care

America’s veterans have done everything asked of them in their mission to serve our country and we believe it’s never too late to give them a hero’s welcome home. That’s why our Hospice is taking part in the We Honor Veterans program. Our staff understand the unique needs of veterans and are prepared to meet the specific challenges that veterans and their families may face at the end of life. We have embraced our mission to serve America’s veterans. It’s our way of saying thank you for the sacrifices they have made in serving us.

We Honor Veterans, a program of the National Hospice and Palliative Care Organization and the Veterans Administration, is designed to help meet the unique needs of dying veterans. The program utilizes respectful inquiry, compassionate listening, and grateful acknowledgement — to comfort patients with a history of military service and possible physical or psychological trauma.

U.P. Home Health & Hospice is proud to partner with the Veterans Administration to offer care to our country’s veterans.

  • U.P. Home Health & Hospice is honored to serve our veterans and their families during their Hospice journey. Examples of the activities that we coordinate for our veterans include:
  • Veteran Screening: Upon entrance to hospice, patients are asked if they have ever served in the military, which branch, during war time or peace, and their years of service. This information is then placed in their file.
    • Train our staff with Veteran Specific Care
    • Caring for our Veterans- A guide for Seriously Ill Veterans, their Families, and Caregivers.
    • Annual Education to our entire staff
  • Veterans Day Activities: For Veterans Day our team coordinates with the patient and family for an appreciation ceremony that includes:
    • Presentation of an engraved watch by our Veteran Volunteer, which highlights their years of service.
    • Lunch provided for veterans and family members
    • Certificate of Appreciation for their service and sacrifice.
  • In the Community
    • Veterans Day Sponsored Activities: In conjunction with Lake Superior Life Care & Hospice, U.P. Home Health & Hospice sponsors Veteran’s Day Activities like pumpkin cheesecake and Big Bucks Bingo for residents of the Jacobetti Home for Veterans.
    • We conduct employee and community wish list drives for the residents of Jacobetti Home for Veterans.
  • Veteran Volunteer: We have a veteran volunteer who provides companionship, respite, and presents the watches during the Veterans Day Ceremony
  • Coordination of Care
    • D.J. Jacobetti Home for Veterans staff, providers, family, caregivers, and skilled nursing team
    • Post death survivor and burial benefits coordinated with the Veterans Benefits Administration
