• Nursing care to educate and assist an individual with a new diagnosis or to monitor exacerbations of a long-term illness such as diabetes, cancer, or heart and lung diseases
• Rehabilitation after surgery, stroke, or injury
• Support for every-day tasks like bathing or other activities of daily living
• Medical Social Workers to help coordinate everything from obtaining a sturdy walker or referral to the appropriate community organization to help sort out a disability benefit
